The employee will receive half after completing 6 months of employment and the remaining half after 1 year of employment.
A case manager is responsible for referrals, needs assessment, care plan development, outreach, and implementing and tracking services for individuals. Must be willing to be trained to administer medications. This position will be discussed in more depth during the interview.
Requirements:
Must possess at least a bachelor's degree.
1+ years of experience preferred, but new graduates will be considered.
Current/valid non-expired driver's license. This position requires driving. To meet the driving requirements, you must be at least 21 and have an insurable driving record.
Social security card.
Ability to demonstrate computer skills in Word processing, Excel, Outlook, and type 40+ wpm.
Completion of MAC (Medication Assistant Certified) training.
Orientation is required for employment, consisting of 5 days of paid training Monday-Friday, 8 a.m. to 4 p.m.
