• Maintaining client confidentiality and safety is priority.
• Assessing clients’ physical and mental wellness, needs, preferences and abilities and using these to develop tailored trauma-informed treatment and person-centered care plan.
• Manage a designated caseload to coordinate and complete timely assessments, planning, implementation and evaluation of therapeutic services and resources as prescribed the person-centered plan.
• Perform case management duties including care coordination/transition and discharge planning, consultations, advocacy, education across the continuum of care.
• Working with clients, support networks and health care professionals to put care plans into place.
• Listening to clients’ concerns and providing counseling or intervention as required.
• Recording clients’ progress including charting referrals, home visits, and other notable interactions
• Evaluating clients’ progress periodically and making adjustments as needed to improve outcomes
• Following up with discharged clients to ensure they were satisfied with services, and they are still in good physical and mental health.

a. Tue - Friday 9a-5:30p
b. Sat -10a-6:30p
Requirements:• Must be able to pass a background check and drug screening as required by regulatory standards
· Minimum 21 years old.
· Experience working with children, adolescents, young adults who have experienced complex trauma.
· Bachelor’s degree in the Human Services, Social Work, or similar field.
· 2 years’ experience in Crisis Intervention, Crisis Intervention Team (CIT), or similar stabilization methods.
· Ability to manage stressful situations with professionalism and decorum
· Knowledge of medication administration and adherence to healthcare protocols and procedures
· Knowledge of federal and state regulations specific benefits and reimbursement guidelines, including denial prevention.
