Case Manager - Urology - FT Days
UCI Health is the clinical enterprise of the University of California, Irvine, a leading academic health system located in Orange County. UCI Medical Center is a 459‑bed acute care hospital that provides tertiary and quaternary care and is home to the region’s only National Cancer Institute‑designated comprehensive cancer center, a high‑risk perinatal/neonatal program, an American College of Surgeons‑verified Level I adult and Level II pediatric trauma center, and a gold‑level 1 geriatric emergency department.
Position Summary: The incumbent will provide coordination and oversight for effective delivery of subspecialty care to patients in the outpatient Urology clinic. The case manager will serve as a single point of contact for patients and/or family members, effectively communicate the patient’s plan of care, and coordinate care with physicians and other members of the healthcare team.
Responsibilities
Coordinate patient care in the outpatient Urology clinic.
Serve as primary contact for patients and families, communicating care plans.
Collaborate with physicians and multidisciplinary team to optimize patient outcomes.
Develop and implement patient and family education programs.
Maintain accurate documentation using Word, Outlook, and Excel or similar database software.
Required Qualifications
Current California RN license and BLS certification.
Minimum 1 year experience as an RN or 3 years MA/LVN experience in a clinical setting.
Experience coordinating care for patients with complicated medical diagnoses.
Strong written and verbal communication skills in English.
Computer proficiency with Word, Outlook, and Excel or similar database software.
Ability to maintain a work pace appropriate to the workload.
Ability to establish and maintain effective working relationships across the Health System.
Bachelor of Science in Nursing for all external candidates.
Preferred Qualifications
Specialty nursing certification related to patient population.
Previous experience as a nurse navigator (care coordinator) for patients with complicated medical diagnoses in urology or pulmonary and/or nephrology clinics.
Knowledge of University and medical center organizations, policies, procedures, and forms.
