Your Role on the Team
Position Summary
Incumbent will provide coordination and oversight for effective delivery of subspecialty care to patients in the outpatient Urology clinic. Incumbent serves as a single point of contact for patients and/or family members for the purpose of effectively communicating the patient’s plan of care, and coordinates care with the physicians and members of the healthcare team.

What It Takes to be Successful
Required Qualifications

Strong written, oral communication, as well as effective problem solving and negotiation skills

Must possess the skill, knowledge and ability essential to the successful performance of assigned duties

Must demonstrate customer service skills appropriate to the job

Minimum 1 year experience working as an RN or 3 years MA/LVN experience in a clinical setting

Experience coordinating care for patients with complicated medical diagnosis

Excellent written and verbal communication skills in English

Current California RN license

Current BLS certification

Computer familiarity using Word, Outlook and Excel, or similar database software

All external candidates must have a Bachelor of Science in Nursing

Ability to maintain a work pace appropriate to the workload

Ability to establish and maintain effective working relationships across the Health System

Ability to design and implement patient and family education programs

Preferred Qualifications

Specialty nursing certification relating to patient population

Previous experience working as a nurse navigator (care coordinator) for patients with complicated medical diagnosis, preferably urology

Previous experience working as a nurse navigator (care coordinator) for patients with complicated medical diagnosis, preferably pulmonary and/or nephrology clinics

Knowledge of University and medical center organizations, policies, procedures and forms
