POSITION TITLE: Case Manager
REPORTS TO: Program Director
DEPARTMENT: Behavioral Health: Ruth Kelley Ummi’s Program
SCHEDULE: Monday–Friday, 9 am–5 pm
The Case Manager will work in the Ummi’s Family Sober Living Program, ensuring appropriate linkages with community services and facilitating comprehensive assessments, care planning, advocacy, and collaboration with multiple providers to help families achieve permanent housing and social/economic stability.
Responsibilities
Provide comprehensive assessments and individualized family service plans.
Advocate and collaborate with providers to navigate and access resources for housing and stability.
Maintain accurate records, submit required reports on time, and network with families, physicians, courts, and community agencies.
Accompany families into community settings when necessary to access services.
Work flexible schedule, including weekends and holidays as required.
Minimum Qualifications
Bachelor’s degree (B.A. or B.S.) or a high school diploma with 5 years of related experience in case management, housing, substance disorder, trauma field.
Experience: 2 years of supervised full‑time work with families with histories of substance use disorders/trauma, or equivalent qualifications (e.g., LADC II). At least 5 years of case management or counseling experience may substitute for degree.
Excellent verbal and written communication, interpersonal skills.
Knowledge of substance‑use disorder and trauma‑informed care, homeless and housing issues, community resources, housing subsidies.
Valid driver’s license and willingness to drive in Boston and surrounding communities.
Ability to work independently and as part of a multidisciplinary team, work flexible hours, including evenings.
Computer literacy; strong customer focus.
