Job Summary
We are a fast‑paced Community Based Mental Health program seeking a compassionate, organized, independent worker experienced in working with individuals who have a mental illness.

Work Hours
8:00 AM – 4:00 PM, Monday through Friday

Key Responsibilities

Develop a treatment relationship with consumers and formulate treatment plans.

Provide psychosocial rehabilitation, symptom monitoring, and supportive counseling to consumers on caseload.

Monitor client’s psychiatric medication regimen and compliance as directed by the physician.

Provide crisis intervention and supportive counseling to consumers who are in crisis.

Maintain a treatment‑oriented environment and assure consistency in carrying out treatment objectives under supervision.

Coordinate utilization of community resources, day programs, and outside health‑care providers.

Assist clients in financial planning and development of income resources.

Coordinate consumer access to services and all entitlement programs.

Provide treatment‑related education and liaison with client’s family.

Assist client in securing adequate housing and provide skill training to maintain housing.

Qualifications

Bachelor’s Degree in a behavioral science (Social Work preferred); Master’s Degree encouraged.

Valid driver’s license and automobile with sufficient insurance.

Background checks, references and driving record meet WCS requirements.

Good computer skills (Word, Excel, CSP Exchange).

Strong organizational skills, ability to multitask and meet deadlines.

Knowledge of mental illness and ability to work with diagnosed individuals.

Team player and ability to work independently.

Other Job Information
Work is performed largely in the community, including client homes, community locations, or other settings. Hours average at least 40 hours a week.

Position is mobile and includes community visits, home visits, and driving throughout Milwaukee County. Ability to lift up to 20 pounds is required.
