Case Manager Supervisor Join to apply for the Case Manager Supervisor role at Community Housing Innovations, Inc. This position leads a team of Case Managers and coordinates resources to support residents in achieving stability and self-reliance.
Responsibilities Develop and oversee individualized service and care plans for residents.
Coordinate partnerships with community-based services and manage service agreements.
Provide direct supervision, coaching, and performance evaluation to Case Managers.
Ensure compliance with CHI code of conduct and state/federal regulations.
Maintain documentation in CARES and AWARDS systems.
Qualifications Bachelor’s degree in social work, psychology, or related field.
Seven (7) years of case management experience and three (3) years of supervisory experience in past ten (10) years.
Proficiency in English; Spanish proficiency prioritized.
Knowledge of CARES, HOME, CAPS, and AWARDS systems.
Strong written and verbal communication skills.
