Schedule: Monday - Friday, 11:00am - 7:30pm
Status: Full-time/Non-Exempt/Non-Management
Job Description Summary Sojourn’s mission is to provide shelter and support services to adult and child victims of domestic violence; prevent violence through non-dominant child-rearing, and advance social justice locally and globally. Services include shelter, support groups, workshops, hotline calls, children’s empowerment programming, legal services, and referrals. Sojourn educates the broader community and supports public policy that affirms the rights of oppressed populations.
Essential Duties and Responsibilities Under the supervision of the Senior Program Manager, the Case Manager will:
Maintain accurate progress notes and documentation of client services.
Adhere to program/contract compliance, confidentiality, HIPAA, safety and crisis intervention guidelines.
Assist with tracking and data entry (daily shift logs, census reports, etc.).
Train, assist and provide feedback to volunteers and Counselor Advocates.
Provide direct services to clients, including:
Case management (housing, medical & social service advocacy, job development, transitional housing, case planning).
Housing Navigation services (advocacy with housing providers, housing search, HUD/EHV navigation).
Possible co-case management with other projects or outside agencies.
Intake/assessments/screenings; assist with crises using trauma-informed de-escalation and mediation.
Implement empowerment model, harm reduction and trauma-informed care.
Crisis intervention, safety planning, resources and eligibility screenings via Sojourn 24-hour DV hotline.
Assist with client entry/exit of shelter programs and transportation to appointments as needed.
Facilitate support groups for Adult and Child Participants (life skills, DV education, parenting). Provide child-care occasionally.
Ensure adherence to shelter guidelines; provide resources and referrals; establish positive working relationships with clients, staff, partners and stakeholders.
Attend staff meetings, case conferences, trainings, and community meetings as needed.
Take inventory of site needs and submit facility maintenance, IT and supply requests.
Submit daily activity reports to leadership and communicate during shift transitions.
Participate in safety drills and facility checks to ensure county health standards; maintain shelter cleanliness.
Report credible suspicions of child abuse to shelter management.
Other duties as assigned.
Qualifications High School Diploma or GED or equivalent
Trustworthy, honest, and a team player
Ability to supervise shelter participants with dignity and respect; engage participants and troubleshoot problems
Sound decision-making ability related to crisis and safety planning
Ability to work with a diverse population and implement cultural responsiveness
Strong communication skills (phone/in-person) with the public, staff and volunteers
Basic computer literacy; ability to maintain confidentiality
Ability to supervise sanitary conditions across all areas
Willingness to work flexible hours and handle emergencies
Understanding of Domestic Violence survivor needs
Use of own vehicle with valid CA driver’s license, reliable automobile and insurance; acceptable driving record
Experience in social services with direct case management preferred; willingness to receive 40-hour Domestic Violence Training
Preferred Qualifications Bachelor’s degree in a related field preferred
Minimum of two years’ experience in social services or case management; 24-hour shelter experience preferred
Experience in communal living or shelter environments; bilingual Spanish preferred
Work Environment Combination of field and office environment
Regularly sit, stand, bend; occasional lifting up to 35 lbs
Occasional walking/driving to local sites; busy and loud environments; exposure to elements
