Overview
Case Manager Skills Trainer for youth services in Grants Pass Schools
This position is co-located within the school. The position provides outreach and engagement activities to children and families that are in need of behavioral health support and/or are in need of help with accessing other community services. The position teaches youth coping skills, provides social skill development, and assists families in developing skills for effectively supporting the youth; also mentors and promotes the development of life skills such as emotional self-regulation, interpersonal skills, and self-advocacy. The majority of services are delivered in the schools.
Responsibilities
Teaches and mentors youth on a one-to-one basis and may facilitate teaching and mentoring in groups. Works with school personnel and Options to design interventions and strategies that will help students be successful in school and life. Facilitates and coordinates access to needed services and supports within the community. Assists students and their families with accessing local services and supports such as housing, social security benefits, education, physical health care etc. Works in partnership with other Options team members coordinating services with other service providers, and actively participates as part of an integrated treatment team. May drive agency vehicle and transport clients as required. Manages time and caseload effectively.
Qualifications
Bachelor’s (BA/BS) degree in a human services field including education OR three years of experience providing services in a similar setting with individuals who have emotional and/or mental and/or developmental disabilities, OR a combination of education and experience that provides the desired skills, knowledge, and abilities required to perform the job.
Must be a QMHA on the Mental Health & Addictions Certification Board of Oregon (MHACBO) registry.

Candidates must be able to use e-mail, compose documents, save and locate documents electronically, and learn to use our Electronic Health Record (EHR) system. Proficiency in Word and other MS Office applications preferred.
Must pass state-required background and DMV checks; possess a valid OR driver's license and a satisfactory driving record to be approved for driving rights. Candidate must be able to work independently and flexibly, under general supervision.
