Position Summary Provide Case Management services to individuals with intellectual and/or developmental disabilities. Establish positive working relationships, explore options and growth opportunities for each person served, and create and monitor a Person-Centered Plan (PCP). Ensure services meet state-mandated standards and securely connect individuals with ongoing supports.

Essential Duties Advocate for individuals and educate them and their families about services and self-determination
Provide information, referrals, and support for transitioning to appropriate services
Develop and maintain PCPs, document outcomes, and secure ongoing supports
Identify, coordinate, and document eligibility for DDA services; assist with paperwork and waivers
Conduct assessments, develop PCPs, and monitor progress toward goals
Coordinate resources and services to meet PCP outcomes; travel to meet with support staff as needed
Submit required documentation (e.g., RECON, AIR) and communicate with providers about resolutions
Ensure compliance with SCI policies and perform within performance benchmarks
Maintain HIPAA records securely and utilize tracking/time management tools

Education Requirements A bachelor’s degree in a human services field; OR
A bachelor’s degree in a non-human services field with two years of experience in a human services field
Experience Preferred Experience working with people with developmental disabilities
