Overview We are currently hiring for a PRN Case Manager (RN) within the Nursing Administration department here at Aspen Valley Health.
Work Shift: Rotating - 12 Hours
Compensation Range: $48.45 - $77.52; Night and weekend shift differentials up to $6 per hour.
The compensation range listed is an estimate. Final compensation will be determined based on your qualifications including years of experience performing similar work, level of formal education in a relevant field of study, and any professional certifications related to the position.
Responsibilities The RN Case Manager determines the appropriateness and medical necessity for all inpatient, observation, and swing-bed admissions, change in level of care and continued stay.
Verifies all status orders and documentation on patients in the inpatient departments.
Responsible for coordinating discharge to ensure continuity of care.
Leads the Transition of Care Team for a targeted multidisciplinary approach to the discharge process for patients.
Facilitates ancillary services for targeted patients.
Requirements Associate degree in Nursing (ADN).
Unrestricted license to practice as a Registered Nurse (RN) in the State of Colorado, or ability to obtain prior to start date.
Basic Life Support (BLS). May be obtained upon hire.
Preferred Qualifications Bachelor's (BSN) or Master's (MSN) in Nursing preferred.
Bilingual in Spanish preferred.
Experience with EPIC preferred.
