Overview Case Manager - Full-Time at Corona Regional Medical Center located in Corona, CA. Reporting to the Director of Case Management, this position performs daily concurrent medical record review for medical necessity and level of care appropriateness, post-hospital planning, social service case finding and intervention, and quality and risk management related issues. Actively assists physicians and hospital staff in the management of patients by facilitating care through interaction with hospital departments and community services. Assures continuum of care for patients and their families through referrals to other acute hospitals and outside community agencies.

Qualifications MINIMUM ESSENTIAL EXPERIENCE
Two years nursing experience in an acute hospital setting required.
Two years case management experience in an acute hospital within the past year preferred.
Required Licensure / Certifications
Current California RN license required.
Current BLS (CPR) certificate preferred.
