Per-Diem Day Shift Case Manager
Location: Wellington, FL – Wellington Regional Medical Center, a 235‑bed acute care hospital owned by Universal Health Services.
Shift: Day. Type: Per‑Diem.
Responsibilities
Ensure appropriate utilization of hospital services; develop discharge plans in a timely manner.
Recognize signs and symptoms of stroke and call a stroke alert overhead; verbalize stroke alert process.
Work with stroke patients and families for discharge planning and treatment based on needs, condition and prognosis.
Maintain current stroke knowledge by completing annual 1‑hour online training.
Perform discharge planning and utilization chart review for acute care patients.
Assist with case management for acute care patients, ensuring all documentation is accurate and complete.
Qualifications
Registered Nurse with Florida licensure.
2‑4 years of recent acute‑care experience.
Experience in utilization chart review and discharge planning.
Managed‑care experience preferred.
Knowledge of InterQual criteria.
Strong clinical background with excellent interpersonal communication skills.
Basic computer skills.
