Job Description Summary
Come join our Florence, SC Case Management team as a full-time ICU Case Manager RN. The role coordinates the overall interdisciplinary plan of care from admission to discharge, monitors care, and acts as a liaison among patients, families, healthcare personnel, and insurers.

Key Responsibilities
Coordinate interdisciplinary care plans for ICU patients from admission to discharge.
Monitor patient progress, act as liaison between patient/family, healthcare team, and insurers.
Evaluate patient needs and available resources; recommend and facilitate optimal care plans.
Develop discharge plans that provide necessary resources and encourage compliance.
Identify patient care issues; suggest revisions or new clinical pathways to improve quality of care.
Perform utilization reviews as needed.
Education & Experience
• Bachelor of Nursing degree preferred with a minimum of one year of experience.
• RN staff hired on or after July 1, 2013 with an Associate or Diploma in nursing must complete an accredited BSN program within two years and obtain BSN within four years of hire or reclassification.
• Prior leadership experience preferred.
• One year of case management and/or utilization review work experience preferred.
• Case Management certification preferred.
Licensure & Certifications
• Registered nurse in the state of South Carolina or a compact state required.
• AHA Basic Life Support (BLS) or American Red Cross BLS for Healthcare Providers certification required.
• Case Management certification preferred.
