About the Role

We are seeking a dedicated RN to serve as a Case Manager, applying clinical expertise and core case‑management knowledge to coordinate care for patients across the continuum of care.

What You Will Be Doing
Integrated case management: assessment, planning, implementation, coordination, monitoring, and evaluation of patient care.
Collect data from patient/family assessments, interdisciplinary care team input, and medical records to evaluate progress along clinical pathways and develop discharge plans.
Collaborate with patients/families, interdisciplinary care team, medical staff, payers, and post‑discharge providers to facilitate evidence‑based care coordination.
Ensure smooth transitions and appropriate referrals to the next provider of care.
Qualifications
Minimum 2–3 years of clinical nursing experience with a focus on case management and discharge planning.
Strong problem‑solving, multi‑tasking, and project management skills.
Knowledge of regulatory and insurance standards, and proficiency in using evidence‑based guidelines.
Current NYS RN license and a Bachelor’s degree in Nursing or a related field.
CCM certification highly desirable.
Compensation
The base pay scale begins at $140,016.47 and may go up to $157,666.47, based on role scope, experience, and other factors. Additional benefits are consistent with the role and include experience‑based options.
