Registered Nurse Case Manager (RN) – Acute Care | Day Shift
Location: Oakland, CA | Kaiser Permanente
Weekly Pay: ~$3,139/week (40 hours)
Schedule: 8:00 AM – 4:30 PM | Monday–Friday

Job Summary
We are seeking a skilled and experienced Registered Nurse Case Manager to support the Utilization Management and Discharge Planning team in an acute care hospital environment. The RN Case Manager will ensure safe transitions of care, optimize patient resources, and support compliance with required clinical standards.
Responsibilities
Manage a designated patient caseload and coordinate transitions of care
Conduct utilization review and medical necessity assessments
Develop and implement individualized discharge plans
Collaborate with physicians, nursing staff, and community resources
Provide patient and family education regarding care options
Maintain accurate documentation in the EMR system
Support compliance with federal, state, and organizational guidelines
Minimum Requirements
Active California RN license
At least 1 year of recent Case Management or Utilization Review experience
BLS certification
Strong knowledge of discharge planning processes and patient safety
Proficiency with EMR systems (EPIC preferred)
