RN Case Manager, ED
The RN Case Manager in collaboration with other internal and external disciplines directs and coordinates the care of patients from admission to transfer or discharge. This is done with the intentions of cost‑effective utilization of hospital resources, minimizing out‑of‑pocket expenses for patients, providing customer support to our internal and external customers, appropriate utilization of resources, and patient advocacy.
Schedule
Full time, 4 10‑hour shifts with an every‑other‑weekend rotation.
Qualifications
Graduate from a school of nursing (RN Case Manager).
Baccalaureate degree in nursing preferred.
Current licensed registered nurse in the State of Ohio.
Minimum 3–5 years recent clinical experience in an acute care setting; discharge planning or case management experience preferred.
Effective communication skills.
Strong organization skills; self‑directed, flexible, and able to adapt to rapidly changing regulatory requirements.
Excellent clinical assessment and analytical skills.
Computer competency.
Effective interpersonal skills; ability to work across departmental boundaries, facilitate problem resolution, and maintain a professional demeanor in difficult situations.
Knowledge of the functional operations of utilization management, discharge planning, care coordination, and transitions of care.
