Performs and provides professional case management services in the Rehabilitation Center in order to provide for the overall coordination in the delivery of services and treatment planning, ensure and document effective cost management, promote the development of positive relationships, ensure continuity of care post-discharge, assist in facility compliance with all relevant standards, and assist in the development and implementation of new products and services, staff education and training, and good management practices.
