Overview Case Manager Registered Nurse/Behavioral Health - Field in Stafford VA. This Case Manager Registered Nurse/Behavioral Health Clinician position is with Aetna’s Long-Term Services and Supports (LTSS) team and is a field-based role out of Stafford VA or Southern neighboring areas. The requirements are for candidates to hold a VA Registered Nurse license and travel up to 75% of the time to meet with members face to face.
RN/BH Licensed Case Manager is responsible for assessing, planning, implementing and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall wellness. Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member’s overall wellness through integration. Services strategies policies and programs are comprised of network management and clinical coverage policies.
Work Location: This is a work at home/field based role with typical office expectations. The role requires the ability to perform close inspection of handwritten and computer-generated documents and use a PC monitor. Sedentary work involves sitting, talking, listening and typing for extended periods.
Must reside in or near Stafford or southern surrounding counties, Virginia.
Responsibilities Work from home (WAH) out of residence in VA and maintain reliable phone and hard-wired internet service.
Follow leader-determined work schedules (Mon-Fri, 8:00–5:00) and be reachable by phone, videoconference, and email during work hours.
Coordinate field-based case management activities, including in-person member visits and collaboration with health care providers.
Assess member needs, plan interventions, implement care plans, and evaluate outcomes to improve short- and long-term wellness.
Address issues proactively and apply network management and clinical coverage policies.
Required Qualifications RN/BH with current, unrestricted Virginia state licensure.
3+ years of clinical experience (med-surg, behavioral health).
Must reside in Stafford or southern neighboring areas.
Must possess reliable transportation and be willing and able to travel up to 50–75% of the time; mileage reimbursed per company policy.
Virginia Driver's License required.
Preferred Qualifications Case Management in an integrated model preferred.
Bilingual preferred.
Education Associates Degree Required.
Licensing Bachelor's of Science in Nursing preferred; Registered Nurse license in VA in good standing.
Independent Behavioral Health License in VA in good standing.
Technology Skills Proficient in Multisystem Navigation; comfortable using multiple monitors and systems.
Effective multitasking: able to talk and type concurrently while managing responsibilities.
Basic technical troubleshooting: set up and resolve common issues independently.
Digital collaboration and documentation: experienced with electronic documentation and remote collaboration tools.
Highly organized and results-oriented; maintains structure to meet performance metrics.

Clean and efficient workspace management.

Pay Range The typical pay range for this role is $60,522.00 – $129,615.00. This range represents base pay for positions in the job grade. Final offer depends on factors including experience, education, geography, and other relevant considerations. This position is eligible for CVS Health bonus, commission, or short-term incentive programs in addition to base pay.
