Job Opportunity At Huntersville Medical Center

Schedule: Full-time; Monday - Friday 8:30 AM - 5:00 PM with two weekend days in a six-week time period

Location: Huntersville Medical Center - Huntersville, NC; award-winning 151 bed hospital in Huntersville, a family-friendly town in the Lake Norman area.

This role is ideal for an experienced acute care RN looking to work in a patient-facing but non-bedside role within a high-performing, collaborative team. A dynamic full-time opportunity with exposure to a wide range of patient populations.

What You'll Do:

  • Complex discharge planning
  • Collaborate with physicians and other members of the healthcare team to assess, plan, implement, and evaluate proactive/complex/safe discharge plans
  • Collaborate with MSW case managers for guidance with psychosocially complex cases
  • Facilitate coordination of care
  • Accept responsibility, authority, and accountability for management of care
  • Serve as a resource for discharge planning and care coordination for medically complex cases
  • Maintain current knowledge of available community resources/post acute care options

What We're Looking For:

  • Education: Two year/associate degree. Associates Degree in Nursing or BSN, required. Four year/bachelors degree, preferred.
  • Experience: Two or more years healthcare experience, preferred. Case management or related experience, preferred.
  • Licensure/Certification: Current RN licensure in state of employment, required.
  • Additional skills required: Weekend and holiday coverage.
  • Additional skills preferred: Knowledge/competence of Microsoft office products and keyboarding.
