Job Responsibilities and Duties
Assess the areas of the person served’s life where assistance is needed.
Offer and coordinate resources for the person served being supported.
Monitor service delivery and work with providers to ensure the person served’s needs are met.
Develop service plans.
Attend regular meetings with the person served and their interdisciplinary teams.
Case‑note client related work to create billable units.
Salary
Non‑LSW wage: $49,000 per year, salaried for 40 hours per week.
LSW wage: $51,600 per year, salaried for 40 hours per week.

Flex‑time available after the first 90 days.
Up to 3 days per week work from home – based on 90‑day evaluations; eligibility determined after completing 90 days of training in our NE Minneapolis office.
Requirements
Graduated from an accredited four‑year college with a major in social work, psychology, sociology, or a closely related field; or with a major in any field and one year of experience as a social worker/case manager/care coordinator in a public or private social service agency.
One year of experience in education or treatment of persons served with developmental disabilities or related conditions, or a minimum of one course that specifically focuses on developmental disabilities.
Valid driver’s license and acceptable driving record per Pinnacle Services policy; vehicle with current insurance.
Successfully clear local and/or state background checks.
Preferred Qualifications
Possess a Licensed Social Worker (LSW) or higher form of BOSW licensure.
