Role Overview
The Case Manager is responsible for utilization reviews and resource management, discharge planning, treatment plan management and financial management, while also completing medical record documentation. You will report directly to the Director of Case Management and provide social work services as required.
Key Responsibilities
Develop and implement patient‑specific, safe and timely discharge plans.
Perform verification of utilization criteria and management criteria reviews.
Build relationships and coordinate with payor sources to ensure proper reimbursement and promote cost‑efficient care.
Demonstrate compliance with facility‑wide utilization management policies and procedures.
Coordinate utilization management compliance with quality management to meet licensure and accreditation requirements.
Maintain fiscal responsibilities and ensure the department obtains maximum reimbursement for the patient.
Facilitate multidisciplinary team meetings, including physicians, nurses, respiratory therapists, and rehabilitation therapists.
Minimum Requirements
Current licensure in a clinical discipline as RN, LPN, LVN, Respiratory Therapist or licensed Social Worker per state guidelines.
Previous RN/LPN/RT/SW/CM experience in an inpatient hospital setting with critical care/acute care patients (e.g., ICU, step‑down, med‑surg).
Adequate experience in acute medical case management and confidence managing a plan of care for chronically critically ill populations.
Preferred Qualifications
Specific experience in care management and discharge planning.
Working knowledge of the insurance industry and government reimbursement.
Availability to work a minimum of two shifts per month on an as‑needed basis (weekdays approximately 8 a.m.‑5 p.m.).
