Case Manager / PRN (RN/RT/SW/LPN)
Location: Select Specialty Hospital
Position Overview
We are looking for a highly motivated Case Manager to provide critical care and discharge planning services in a critical illness recovery setting. The role requires licensure as a Nurse (RN/LPN/LVN), Respiratory Therapist, or Social Worker and experience in an acute/critical care environment.
Shift Information
• Availability to work a minimum of 2 shifts per month on an “as needed” basis, scheduled on weekdays approximately 8 a.m.–5 p.m.
Responsibilities
Develop and implement patient‑specific, safe, and timely discharge plans.
Perform verification of utilization criteria and conduct utilization reviews.
Build relationships with payor sources to ensure proper reimbursement and promote cost‑attentive care via resource management.
Demonstrate compliance with facility‑wide Utilization Management policies and coordinate UR compliance with Quality Management.
Maintain fiscal responsibilities, identifying and negotiating reimbursement to maximize insurance benefit coverage.
Facilitate multidisciplinary team meetings with physicians, nurses, respiratory therapists, and rehabilitation therapists.
Ensure accurate medical record documentation.
Qualifications
Current licensure as a Nurse (RN/LPN/LVN), Respiratory Therapist, or Social Worker (per state guidelines).
Prior inpatient hospital experience in critical or acute care settings (ICU, step‑down, med/surg, ventilator).
Adequate experience in an acute medical case management setting, confident in managing and directing a plan of care for chronically critically ill populations.
Experience in care management and discharge planning preferred.
Working knowledge of the insurance industry and government reimbursement.
Preferred Qualifications
Specific experience in Care Management and Discharge Planning.
Availability to work a minimum of 2 shifts per month on an “as needed” basis (weekday 8 a.m.–5 p.m.).
