Background
Washington Masonic Services (WAMS) is an independent 501(c)3 nonprofit corporation that works closely with the Masonic Grand Lodge of Washington. WAMS’ mission is assisting Masons and others in need, focusing on aging in place and securing resources for essentials; investing in youth and education, boosting their ability to achieve their greatest potential; and preserving and curating Masonic heritage, illuminating Masonry’s impact in Washington’s past, present and future. This mission is achieved through statewide programs and services, primarily for older adults, also including youth scholarships as well as the operation of the Masonic Library & Museum.
This position will be part of a collegial team of eight talented staff who partner with a dedicated 12-member Board of Trustees, Advisory Committee members, and other volunteers. We strive to strengthen the personal leadership of each other and those we serve across Washington, expanding each individual’s hope, agency, and dignity. Employees work both in-person and remotely throughout Washington state.
Summary of Position Responsibilities
The Case Manager position primary responsibilities are: Helping older adults lead meaningful and dignified lives by supporting their efforts and goals to remain independent at home, or at home with volunteer or in-home care. The Case Manager works directly with older adults and their families facilitating their access to needed benefits and services. Activities include conducting intake, home visits to perform a needs assessment, stabilization, developing and implementing a personalized care plan, arranging for and coordinating appropriate supports, ongoing monitoring, advocacy, and evaluation. Assisting non-Masonic community members address critical life needs that cannot be met through other resources. The Case Manager will also interact and engage with Masonic Lodges, Masonic affiliated groups, and community resources.
The individual in this position plays a key role in three ways.
Providing direct services to individuals in need.
Building and growing relationships with Masonic and community partners for the purpose of bringing support and charitable relief to the individuals we serve.
Expanding reach and impact statewide.
The Masonic Outreach Services Case Managers provide direct services in the following areas:
Information & Referral – Help callers connect with community resources.
Charitable Relief – Coordinate community and Masonic resources to help people meet their basic needs for food, clothing, shelter, utilities, communication, medicines, and transportation.
Independent Living for Seniors
Long-term Care – Care planning, coordination and management of transitions from independent living to long term care.

The Case Manager, under the supervision of the program director of their designee, will also interact and engage Masonic Lodges, Masonic affiliated groups, and community resources to assist.
CLIENT SERVICES & SUPPORT DUTIES
Provide prospective clients, clients, client families, and/or client personal representatives with an excellent customer experience, exhibiting warmth, friendliness, knowledge, compassion, dignity and respect.
Conduct new client intake over the phone, schedule and perform home visit to conduct needs assessment, develop a personalized care plan, and monitor care.
Collaborate with clients, client families and/or client personal representatives to develop, prepare, and implement written personalized care plans for clients.
Assist in connecting clients with service providers and in applying for benefits and to obtain resources.
Advocate for clients with outside agencies, including governmental agencies, private providers, and third-party payers.
Reassess clients’ needs through home visits and visits with client families, and/or client personal representatives.
Maintain confidentiality of all client information at all time.
COMMUNITY EDUCATION OUTREACH DUTIES
Represent Washington Masonic Services publicly at all times with a high standard of knowledge, skill, professionalism, ethics, and friendliness.
Provide occasional information talks and presentations as requested.
ADMINISTRATIVE DUTIES
Utilize computers, tablets, smart telephones, the Internet, software, keyboards, mice and other input devices independently to:
Update and maintain information & referral resource databases and records.
Maintain detailed electronic documentation of client records.
Perform basic clerical functions, such as preparation of correspondence, e-mail, calendars, tasks, notes, newsletters, etc.
Record and track accurately all client interaction and support as it occurs.
Use the Internet, search engines and other electronic resources to obtain information necessary to conduct client, outreach, or ordinary business work.
Be able to independently use pens, pencils, paper, computers and associated software.
Devise, prepare, and issue plans, narratives, reports according to deadlines and assignments, with minimal supervision.
Fully complete all organization forms per policy and guidelines used to conduct business. These forms include, but are not limited to timesheets, purchase authorization request forms, mileage forms, etc.
Meet or exceed all deadlines with high quality of performance.
TEAM DUTIES
Actively participate in and contribute to organization all-staff meetings and retreats, department meetings, and other meetings as assigned.
Contribute to an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the organization and in the community.
Develop and maintain positive relationships with clients, their families, partner agencies, and staff.
ESSENTIAL FUNCTIONS
Receive direction and comply with instructions from supervisor.
Be able to travel within assigned region. This travel will require long periods of sitting and operating a motor vehicle. Occasional overnight travel will be required for meetings and activities.
Engage in extensive face to face interaction with clients, community members, collaborating agency employees for the purpose of intake, assessment, planning, monitoring, direct support, and evaluation.
Speak in front of and present information to audiences, large and small both in person, on the telephone, and using virtual meeting/video tools. These include Masonic, Masonic affiliated, and community groups.
Ability to conduct extensive operation of office computer, laptop, and mobile technologies for the logging of client data, preparation of correspondence, and provision of education outreach activities such as PowerPoint presentations, webinars, and teleconferences. This includes the operation and occasional troubleshooting of both hardware and software and related issues.
Preparation of reports, correspondence, and other business or client documents with a high level of accuracy and proper grammar and usage of the English language.
Ability to travel, occasionally overnight, for client visits, to attend meetings, and to conduct information sessions or workshops.
Adhere to the policies and procedures of the organization at all times.
Provide excellent customer service for clients, stakeholders and community members using a positive attitude and friendly demeanor.
Problem solving and addressing concerns directly with the supervisor using verbal and written communication when challenges, issues, or concerns about the work, the position, or the organization arise.
QUALIFICATIONS
Demonstrated academic training, knowledge and experience with human services, social work, senior and veteran information & referral, case management, advocacy and resource-related issues.
Must be knowledgeable and skilled in care coordination processes including assessment and service plan development and implementation.
Must be knowledgeable and skilled in using information & referral systems.
Must be able to practice attention to detail and effective self-direction to meet program goals and accomplish the essential functions of this position with high quality.
Must be able to give verbal presentations to small and large groups.
Must be able to use and operate a personal or company issued vehicle for the purpose of traveling to home visits, meetings, monitoring personalized care plans, or for other purposes as assigned.
Must be able to operate laptop computers, presentation software, and computer projection systems.
Must have demonstrated skill in the use of PC computers, including use of the internet, search
engines, email, electronic calendars, tasks, notes, word processing, spreadsheet creation and analysis, database use and report writing.
Must be able to read and write in the English language at least at bachelor’s degree or equivalent level.
Must be skilled in and experienced with providing written reports, narratives, and information using the English language.
Must be able to operate and troubleshoot standard office equipment including computers, copy/fax/scan machines and postage meters.
Must have excellent interpersonal skills.
EDUCATION
A master’s degree in social work (MSW) is preferred. A bachelor’s degree in social work, human services, health, or related field may be accepted with a minimum of 3-5 years practical experience in the field of older adult services or geriatric care management.
This position is subject to Washington State Patrol Background Check and/or other background checks on an annual basis. Individuals who have been arrested for or convicted of crimes against children, seniors, or other vulnerable populations are not eligible for employment and will be dismissed if an arrest or crime of this nature occurs once employed.
