Case Manager
Program & Location: Transitional Support Services (TSS), New Bedford
Education & Experience: Minimum of High School Diploma or equivalent, Associate's degree or higher & supervised counseling experience in substance use treatment or closely related field preferred.
Pay Range: Diploma (Clinician 3) $22 (experience/education dependent)
Bachelors/LADC 2 (Clinician 2) $28 (experience/education dependent)
Masters (Clinician 1) $34 (experience/education dependent)
Status/Shift: Part Time - Saturday and Sunday - 9:30am-6:00pm
Hours: 16 hrs a week

About Us
Our Transitional Support Services Programs (TSS) offer services that bridges the gap between detox and step-down services to residential after-care.
High Point & Affiliated Organizations is a health and human service agency whose mission is to treat and prevent substance use disorders and mental illness. High Point has programs located throughout Southeastern Massachusetts offering a full continuum of care for substance use and mental health treatment, including inpatient, outpatient, residential, and community-based services. Programs and services also assist survivors of abuse, violence, and families experiencing homelessness. High Point believes that everyone has inherent goodness, worth, and dignity. Our goal is to help individuals and families achieve personal change and improve their quality of life.
Case Manager Requirements
Must present as courteous and professional at all times
Demonstrated flexibility and ability to perform multiple tasks
Must have excellent communication skills, including the ability to organize ideas in logical and
clear fashion
Knowledge of basic computer skills and software programs
Must have the ability to work with others in cooperative and collaborative manner
An understanding of the relationship between substance use and mental health issues and/or self help programs
Understand and maintain therapeutic boundaries
Reinforce rules in a respectful manner
Comply with Federal Regulations, 42 CFR Part 2, Confidentiality of Substance Use Disorder and
HIPAA Regulations
TB/Flu and Covid19 Assessment
Certification in the HPAO approved de-escalation program
CPR Certification
Successful completion of all trainings upon hire and annually as required
Case Manager Duties & Responsibilities
Maintain safety and security of residents / facility
Ensure timely contact of Program Manager/Authorities in event of a crisis/emergency
Understand and follow established referral procedure and schedule intakes
Facilitate intakes of new residents including setting up new record and completion of intake
paperwork
Completion of Initial Individual Service Plan (ISP) with resident
Referrals for needed services to appropriate agencies/providers including but not limited to: DTA,
SSA, DCF, Medical/Mental Health agencies, etc.
Maintain accurate case files, ensures required Authorizations to release/obtain information are
signed by residents, prepares legible case management notes of each session, documents referrals on designated form, updates ISP’s monthly and ensures periodic evaluations are done timely
Accurately record resident attendance at meetings, appointments, groups, etc.
Meet with each resident on a bi-weekly basis to review/evaluate progress in treatment/recovery to determine if current needs are being appropriately and satisfactorily met; to revise/update the
resident’s Individual Service Plan as indicated, including the establishing of additional
goals/objective and the steps to achieve these goals/objectives
Attend monthly case review with Program Manager
Attend clinical supervision, required staff meetings and trainings
Ensure timely completion of Admission/Discharge satisfaction surveys by residents
House driving/transportation
All other duties as assigned by the Program Manager
Case Manager Qualifications
High School Diploma or GED and one-year experience in substance use treatment field preferred
Valid Driver’s License
