Overview
We are pleased to offer the opportunity to work as a Case Manager within the DuPage County Department of Community Services. Case Managers provide comprehensive case management and/or service coordination in a variety of areas. The current Case Manager opening is in the following program/assignment area:

Long Term Care Ombudsman
Case managers in this program represent and advocate for residents, age 60 and older, in long term care facilities, licensed assisted living and supportive living facilities. They receive, investigate, and attempt to resolve concerns and complaints regarding the care of residents as well as providing a regular presence in the licensed facilities.

This is a grant funded position.

Responsibilities

Identify, investigate, and seek to resolve complaints on behalf of long-term care residents

Provide information and assistance, and community education

Maintain a regular presence in long-term care settings

Support and assist in the development of resident and family councils in LTC settings

Provide follow-up and monitoring

Complete all mandated paperwork with accuracy and in a timely manner

Utilize various computer programs for daily tasks and reports

Perform other related duties as required

Requirements

Completion of a B.A. or a B.S. Degree in Social Science, Human Services, Social Work, or a related field or be a RN or LPN

One (1) year of experience in assessment or service provision

Must possess and maintain in good standing a valid Illinois Driver’s License

The preferred candidate will:

Be fluent in a foreign language

A pre-employment background check is required.
