Case Manager Coordinates service needs and collaborates with other services and agencies. Completes assessments and determines the appropriate level of services for new members served.
Shifts Available: AM / PM / NOC / Varies as needed. Full Time, Part Time, 1 or 2 Day per Week Available . 32 hours per week, 36 hours per week, 40 hours per week = must be willing to work either Saturday or Sunday as one of the days in the shift.
What You Bring To The Table (Must Have):
Master’s degree in Social Sciences
One (1) year of experience in providing crisis management and mental health services
Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
Willingness to use the company vehicle to drive clients to appointments and groups, etc.

Telecare behavioral health program is a 24/7, Mobile Crisis Response Team (MCRT) serving individuals of all ages experiencing mental health crisis.
MCRT is creating an opportunity for to intervene in a unique way that has never been done before in our system of care.
This program collaborates and accepts referrals from the Access and Crisis Line (ACL) and law enforcement.
MCRT will offer crisis triage and management, risk assessment, in-person intervention, and case management.
Each team will respond to calls from the mobile crisis van to provide services directly in the field
