We are looking for an experienced Case Manager LVN - Full-time. We offer FREE cell phones w/unlimited talk/text/data plans to all full time employees (restrictions apply).
The Case Management Department is responsible for promoting the efficient utilization of skilled services by ensuring that patients are admitted according to medical necessity. Services are provided to meet the assessed need of each patient in a prompt and efficient manner, and in accordance with patients “insurance”. The department facilitates timely discharge to the most appropriate environment with necessary supportive services to meet ongoing care needs and ensures proper authorization of care and level of service.
