Job Summary
Odyssey House Louisiana, Inc. is seeking a full-time Case Manager . The Case Manager will determine eligibility for guests to participate in the Low Barrier Shelter (LBS), assess guests' needs, and develop effective housing plans. Responsibilities include meeting with guests regularly, assisting with applications for Medicaid and other documentation, entering data into HMIS, and coordinating transportation and discharge planning. The role requires preparing reports, attending meetings, and providing crisis support as needed.
Responsibilities and Duties
Document services and provide case management to approximately thirty or more guests.
Screen, assess, develop housing plans, monitor progress, and link guests to community resources.
Assist guests with obtaining vital records, IDs, and signing lease agreements.
Establish partnerships with housing agencies.
Assess and maintain guests' Medicaid status.
Enter and update guest information in the agency’s database.
Prepare reports and maintain occupancy records.
Utilize Motivational Interviewing techniques.
Refer guests to vocational, educational, housing, and social services.
Attend meetings and training sessions.
Verify assistance outcomes and provide crisis management when needed.
Complete incident reports and maintain licensing requirements.
Oversee daily guest activities and perform other duties as assigned.
Required Qualifications and Skills
Be over 18 years of age.
Excellent communication, organizational, and interviewing skills.
Ability to work with guests from diverse backgrounds and with disabilities.
Bachelor's degree in social work/behavioral sciences or three years relevant experience.
Valid Louisiana Driver’s License with a clean driving record.
Ability to operate an eight-passenger van.
Preferred Qualifications
Personal experience of being unhoused.
At least three years of case management experience with the target population.
Experience managing databases.
