About the Role
As a Case Manager / Job Coach , you serve a critical role in engaging potential clients in suitable career paths. You'll work one-on-one with clients to offer them community resources, assist them in overcoming barriers, prepare them for employment, help them navigate job searches, and support their long-term success in the workplace. You will support the program manager and collaborate with other team members and community partners to ensure seamless services and drive positive outcomes.

Key Responsibilities
Serve as backup to trainers to co-facilitate Info Session for potential new clients
Conduct intake interviews and assess client readiness for Career Focus training
Identify barriers and barrier management methods/resources
Work closely with the Career Focus team to support the client’s success in the classroom
Connect clients with supportive services as needed
Communicate with job coach leads after three (3) Career Focus days for coaching assignments
Serve as a coach for Career Focus clients and follow up on clients’ training, placement, and post-placement
Track and contact registered Career Focus clients who do not attend sessions, and follow up with participants who do not register for Career Focus
As needed, provide coaching support for clients in other training programs
Communicate with individual coaches about any gaps in service, data entry, and/or reporting
Assist the training team in fully engaging participants and coaches
Maintain appropriate tracking and reporting data on trainings, clients, etc.
Assist Career Focus manager with communication & administrative tasks as assigned
Attend all direct service and staff meetings
Other duties as assigned
What Success Looks Like
Oversee timely and appropriate services, from outreach, enrollment, to coach assignment
Work collaboratively with all MND staff to offer clients a seamless experience
Meeting or exceeding recruitment, engagement, and placement goals
Consistently high participation and performance outcomes in MND training programs
Develop and maintain an up-to-date resource guide of community partners' services
Continually direct coaches on gaps or needs to support the client’s successful outcomes
Timely and accurate documentation in client tracking systems, updated within one week
Strong relationships with employers, service providers, and community partners
What You Bring
Bachelor degree or equivalent work experience in human services fields
Effective oral and written communication
Experience working with diverse populations is a MUST
Ability to relate to clients and successfully assess barriers, needs, and resolutions
Ability to cultivate collaborative relationships with all MND stakeholders
Strong computer literacy in Microsoft Suites (Excel, Word, PowerPoint)
Flexible with a solutions-oriented attitude
