Position Summary
Case Manager participates as a member of the Jail Transitions program team to intensively support patients by initiating services while incarcerated and to continue services into the community. This position is part of a multidisciplinary team that provides services designed to assist and support an individual diagnosed with or being assessed for a mental health disorder, to gain access to needed medical, social, educational, and other services as well as provide care coordination and care transition services.
Our ideal candidate will have a bachelor’s degree in one of the recognized mental health professions; a master’s degree is preferred. They will also have previous experience with substance abuse and mental health related case management and treatment, with at least one year of human service–related experience preferred.
Salary: Starting pay is $21.00 (Base pay $18.50 + $2.50 locale differential = $21.00) and ranges up to $23.72 per hour, which includes a $2.50 locale differential at our Durango worksites.

Physical Demands
Employee must occasionally lift and/or move up to 15 pounds. Employee is frequently required to walk, sit, stand, or kneel and occasionally required to climb or balance and stoop. Must have ability to sit for longer periods at a computer. Employee must be able to travel between AXIS HEALTH SYSTEM locations. Possible potential exposure to communicable disease. Must have current driver’s license and qualify for commercial vehicle insurance and maintain a driving record that allows insurability with Axis Health System’s insurance agency.

Additional eligibility requirements: Annual Flu immunization, Annual TB screening, CPI training within 90 days of hire, BLS PRO certification required within 90 days of hire.
