Seeking a Case Management Coordinator to collaborate with patients, families, physicians, and interdisciplinary teams to ensure efficient and cost-effective patient care transitions.
Responsibilities Manage patient flow, facilitate safe transitions, and optimize length of stay while enhancing patient satisfaction and safety.
Provide comprehensive assessment, planning, implementation, and evaluation of individual patient needs, including care coordination and discharge planning.
Fulfill payer-based requests as appropriate, adhering to organizational policies and regulatory standards to ensure quality care.
Qualifications Nursing degree (BSN preferred) or Master’s in Social Work (MSW) with valid licensure in New Mexico.
Case management certification (ACM, CCM, RN-BC) is highly desirable.
Effective communication, teamwork, critical thinking, resource utilization, multitasking, and organizational skills, with knowledge of community resources.
Preferred experience: at least one year in an acute care setting.
Additional Details Reports to the Director of Case Management; works in hospital and office environments with mobility around hospital units.
Must be able to move frequently, have good vision and dexterity, and work in a fast-paced environment.
