Case Manager - Infectious Disease Join HHM Health as a Compassionate Case Manager focused on HIV, Hepatitis C, and STI treatment. You will coordinate care, engage patients, and advocate within a patient‑focused, culturally sensitive environment.
What you’ll do Act as the primary patient contact via phone and in‑person, establishing professional, compassionate relationships.
Monitor patient care, track referrals, manage scheduling, and address social determinants of health with appropriate resources.
Collaborate weekly with the Care Team to promptly address patient needs.
Schedule new patients, conduct reminder calls, and manage the infectious disease queue.
Facilitate hand‑offs from external providers via phone, fax, or email.
Maintain detailed documentation of patient encounters and meet reporting requirements.
Track patient attendance at appointments, initiate follow‑ups for missed visits.
Participate in outreach testing, share educational resources on HIV, Hepatitis C, and STIs, and provide PrEP education when requested.
Represent HHM Health at professional conferences, in‑service training, and meetings as directed.
Qualifications Licensed Bachelor or Master of Social Work (LBSW/LMSW).
BLS certification.
Bilingual fluent in Spanish is a plus.
Strong understanding of social determinants of health and community resources.
Strict confidentiality compliance with HIPAA and organizational policies.
Proficiency in patient tracking and documentation systems (e.g., eClinicalWorks).
Excellent communication and relationship‑building skills.
Comfort discussing PrEP and other sensitive topics with patients.
Flexibility to work on special projects and occasional weekends as needed.
