Position Overview
The Case Manager II, Registered Nurse - Population Health Medicaid is responsible for providing medical case management services to a specified panel of patients within the clinical setting. This position offers a 40‑hour workweek, no weekends, full benefits package, 12 paid holidays, bi‑annual bonus, and bi‑weekly pay.

Essential Tasks

Provide medical case management to medium/high‑risk patients.

Perform annual comprehensive assessments to evaluate patients’ health care needs and social determinants of health to develop individualized and person‑centered care plans using a collaborative approach including patient and family participation where possible.

Collaborate with community agencies for referral resources and utilize NC Care 360.

Establish and document a care plan with the patient and/or family members. Update and document care plans as the patients’ needs change and/or to address gaps in care.

Identify patients with recent emergency department visits or hospital admissions, and assist them with transitioning back to their primary care provider, including medication reconciliation and assisting with medication access.

Experience and Education

Graduate of an accredited school of nursing as a Registered Nurse.

Current and unrestricted license to practice in North Carolina.

Previous experience, preferably, in a community health center setting in the area of nursing and/or case management.

Must demonstrate the skills and judgment necessary to provide direct care to patients and nursing interventions under the direct supervision of providers.

Strong computer skills and working knowledge of the Windows‑based environment and Electronic Health Record (EHR) experience.

Active North Carolina Driver License.

Must have a personal vehicle readily available for frequent travel between hospital, patient’s home, and site‑to‑site visits.

Effective communication and interpersonal skills.

Schedule

Monday, Tuesday, Thursday, Friday – 8 am – 5 pm (8‑hour day shift)

Wednesday – 11 am – 8 pm (8‑hour evening shift)

Physical Requirements

Constant standing and walking, occasional bending, stooping, lifting, reaching forward and overhead.

May occasionally be required to lift patients, office equipment, and paper, usually not exceeding fifty pounds of direct lift.

Frequent computer work required.

Base Pay Overview
The starting pay for this position is $31.50 hourly. Please keep in mind that actual salary will vary based on various factors such as qualifications, skills, competencies, and proficiency for the role.
