They may help individuals with mental illness and co-occurring disorders live as independently as possible in their chosen community under direct supervision as part of the Case Manager I or Case Manager II role.
Responsibilities Under direct supervision, serve as a primary support and case manager for individuals with mental illness and other co-occurring disorders to promote independent living in the community.
Shifts and Compensation Shifts Available: Short-Hour 0.4 | PM | 3:00 PM - 11:30 PM | Sunday - Monday
Expected starting wage is $22.95 for Case Manager II. Pay ranges are based on geographic location. Starting pay is commensurate with relevant experience above the minimum requirements.
Shift differentials: 6% for PM Shift; 10% for Overnight Shift. Weekend differentials: 5% for Weekend AM; 11% for Weekend PM; 15% for Weekend Overnight.
What You Bring to the Table (Must Have) High School Graduation or G.E.D. equivalent; OR High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; OR Associates degree and one (1) year of experience; OR Bachelor's degree in Social Services
Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
Willingness to use the company vehicle to drive clients to appointments and groups, etc.
