Case Manager II, Enhanced Care Management
Family Health Centers of San Diego is looking for a Case Manager II. Under limited supervision, performs intensive, therapeutic case management services to assigned individuals and supports planning, development, implementation, and evaluation of services in accordance with contract and department requirements.
Responsibilities
Completes all required documentation accurately, timely, and in accordance with department standards; assists in preparing reports as required.
Conducts initial and ongoing assessment of client health and/or support service needs; determines level of client need.
Creates formalized case plans and goals with high-need clients; develops a written care plan identifying problems, needs, interventions, and measurable goals; updates plan as client needs change.
Performs other duties as assigned.
Provides therapeutic interventions and intensive individual support; may include referrals to internal and community services, accompanying clients to housing services, appointments, and social services; focus on medical adherence and appointment maintenance.
Qualifications
3 years of work experience providing human services to high risk, medically underserved, disabled populations, and/or relevant community health populations required.
Bachelor's degree in Social Science, Public Health, Healthcare Administration, or closely related field required. Master's degree preferred.
Or equivalent combination of education and experience meeting essential job duties and any required state or federal certification requirements.
For AOD Services Department (159): Current American Heart Association healthcare provider CPR (BLS) or related advanced certifications.
Ability to travel within San Diego County; active Class C California driver's license with required insurance; acceptable driving record per policy.
Ability to work well in team and independent settings.
Basic computer literacy for EMR documentation and related tasks.
Basic counseling, active listening, and paraphrasing skills.
Strong organizational skills, attention to detail, time management, and ability to meet deadlines.
Multilingual preferred (English, Spanish, Arabic, Somali depending on priorities).
Cultural sensitivity and respect for diversity; excellent interpersonal and communication skills.
Knowledge of San Diego community resources and ability to establish working relationships with community partners.

Additional Information
