Statement of Purpose
Initiate, complete, and follow up on the admissions, assessment, and referral process. Communicate professionally, obtain accurate information, and make appropriate placements in BHS facilities and referrals to other treatment programs. May be assigned to special projects.
Major Tasks, Duties, and Responsibilities
Display advanced knowledge of: drug and alcohol dependency, mental health, family systems; referrals; community resources; and the philosophy of the 12-Step recovery model (e.g., AA, CA, NA, Alanon).
Provide comprehensive reports for assignments and projects.
Complete outcome evaluations as required.
Maintain accurate records and files.
Write clear statements summarizing client feelings, expressions, resistance, and responses to assessments, including letters to referral agencies, courts, and probation.
Maintain and update waiting lists for all treatment modalities.
Confront behavioral changes, identify crises, and recognize when additional assistance is needed.
Interpret and summarize assessment information; provide referrals and treatment recommendations.
Screen for and identify co-occurring disorders; refer to medical, mental health, and HIV/AIDS services.
Conduct searches of personal clothing and property as required.
Conduct urinalysis drug tests and pregnancy screenings as needed.
Ensure all consent forms are completed and signed; complete other registration forms.
Determine client’s fiscal status, verify third-party payer information, and establish fees if applicable.
Demonstrate ethical and professional behavior.
Respect cultural and lifestyle diversities of clients and staff; maintain cultural sensitivity.
Represent BHS positively in all contacts.
Familiarize with and follow BHS policies, procedures, and regulations.
Attend staff meetings and trainings.
Comply with health and safety policies.
Case Manager II Competencies and Performance Expectations
Communicate clearly with clients, staff, peers, supervisors, and community contacts.
Identify personal boundaries, strengths, and limitations.
Adhere to professional boundaries.
Complete documentation thoroughly and timely.
Display intermediate computer skills, including Windows and word processing, and adapt to other software programs.
Prerequisite Qualifications
High school diploma or equivalent; current certification from a credentialing body (e.g., CCAPP, CAADE) upon passing the California Certification exam; two years supervised experience in substance abuse or related field. Certification must be maintained; loss results in employment termination.
Valid California driver’s license and liability insurance if driving a personal vehicle on BHS business.
Good vision, hearing, verbal communication skills, manual dexterity, and eye-hand coordination. Ability to sit at a desk, use a keyboard, write, and move about the facility. Ability to lift up to 10 pounds routinely and up to 25 pounds occasionally, using assistance or tools as needed.
