Overview:PASSages:PASSAges is a program offered by Community Healthlink that provides services within the agency's adult detox services continuum.As a post detox program, the program provides intensive, community based, short term (up to 30 days), residential, group and individual treatment for individuals in the early stages of substance abuse recovery.The program serves adult men and women; length of stay is based on medical necessity.While in the program, individuals gain stability and the necessary skills to assist in transitioning to a substance free lifestyle.In addition to intensive clinical treatment, PASSages also provides case management services to assists individuals with developing comprehensive aftercare plans.Responsibilities:1.Ability to implement program policies and procedures in order to maintain daily structure and consistency within the program milieu and maintain compliance with DPH licensing standards.2.Ability to effectively communicate and develop positive professional relationships with staff and co-workers and outside professionals and complete all required documentation.3.Knowledge of and edibility to work effectively with community based resources, referral networks, and public/private systems of care.4.Ability to assess basic physical, social, educational, legal and service needs of a client and coordinate care with other inter-agency and community based providers.Specific Duties:Work with treatment team to provide comprehensive intakes and assessments to include clinical, legal and additional support service needs for clients.Tobacco Education Coordinator:Duties may include the assignment of Tobacco Education Coordinator.Develop appropriate comprehensive and appropriate aftercare plans while working with clients and members of multi-service team providers.Conduct outreach activities at appropriate referral sites; work closely with other providers to receive referrals, conduct admissions interviews and initial assessments for review by Program Coordinator or Clinical Team.Assist clients in engaging next level of services.Coordinate and provide transportation as needed.Participates in daily multidisciplinary team to discuss client issues.Make referrals and assist clients with transition to behavioral treatment providers including outpatient services, residential placement and self-help groups.Maintain statewide referral database.Lead at least two skill development, educational or recreational group per week as assigned.Attend program staff and clinical team meetings, supervision, trainings and all other required program meetings.Document treatment relevant information in charts (progress notes); assessments, follow up interviews, case conferences, behavioral issues on the unit, coordination with other agencies or programs, referrals, clinical team treatment decisions, discharges and others.Perform other related job duties.Qualifications:-Certification in addictions counseling or minimum of 1 (one) year of experience in field required.-Current valid US-issued driver?s license and ability to provide registered and insured automobile for work related purposes.-Knowledge of the Massachusetts continuum of care for substance abuse.-Understanding of addiction and recovery.-Desire to serve others required.-Demonstrated strong work ethic, ability to work independently, and ability to contribute to team objectives required.-Preference will be given to those with experience in social services and/or working with program?s population.All bilingual candidates are strongly encouraged to apply.Community Healthlink is an Affirmative Action/ Equal Opportunity Employer.We do not discriminate in employment and personnel practices on the basis of race, sex, gender identity, age, ancestry, disability, religion, national origin, marital status, sexual preference, political affiliation or any other basis prohibited by applicable law.Hiring, transferring and promotion practices are performed without regard to the above listed items.If applicable, Community Healthlink shall also abide by the requirements of 41 CFR 60-300.5(a) and 41 CFR 60-741.5(a).These regulations prohibit discrimination against qualified protected veterans and qualified individuals on the basis of disability.These regulations require affirmative action by Community Healthlink to employ and advance in employment qualified protected veterans and individuals with disabilities.Posted by StartWire..
