Case Manager I – Rapid Re‑Housing Program
Father Joe’s Villages is looking for a compassionate Case Manager to provide direct and indirect services to culturally diverse individuals, families, and transitional‑aged youth. The role includes comprehensive assessment, case planning, advocacy, and support services that help clients progress toward self‑sufficiency during and after the Rapid Re‑Housing Program.

Pay Range
$21.82/hr – $28.07/hr (midpoint $24.56/hr). Actual pay will be based on skills and experience.

Essential Functions

Maintains a maximum caseload of 23 households.

Conducts weekly, bi‑weekly, or monthly meetings with each client.

Performs at least one home visit per month, grocery runs, and service provisioning throughout San Diego County.

Completes monthly property‑management engagement to maintain relationships.

Creates and reviews monthly budgets with clients, including the Rental Assistance Plan.

Develops and maintains individualized case plans for each client.

Identifies and connects clients to resources within FJV and the community.

Conducts lease reviews.

Enters case notes and services into the CSTAR database promptly.

Attends weekly multidisciplinary team meetings and presents client updates as needed.

Completes assigned training and policy requirements on time.

Performs other duties as assigned.

Qualifications

Bachelor’s Degree or equivalent education/certifications.

Minimum 2+ years of similar work experience or lived experience.

Valid in‑state driver’s license, current registration, insurance, and a driving record with no more than 2 points in the last 3 years.

Relevant training and/or experience that provides the required knowledge, skills, and abilities for the role.

Professional or personal experience supporting individuals with disabilities, mental illnesses, or challenging behaviors is highly preferred.

Mobility to work in an office setting and use standing office equipment.

Proficiency in typing and Microsoft Office.

Strong problem‑solving, analytical, and prioritization skills.

Excellent interpersonal skills and ability to work with a diverse population.

High level of professionalism with confidential and sensitive information.

Reliable attendance.

Ability to understand and follow written and verbal instructions.

Knowledge of and compliance with HIPAA standards.

Process improvement, data entry accuracy, and strategic planning abilities.
