Job Overview:
The Case Manager facilitates treatment and provides direct practice in the areas of group work, consumer education, treatment planning, individual counseling, and coordination of referrals and intakes as necessary.
Qualifications:
Required:
Bachelor's degree in behavioral health/social service field
1 year behavioral health case management experience
Skills in Training, Crisis Intervention, Group work, Individual and Family counseling.
Preferred:
Previous Partial Hospital Program experience

Day Shift
40 hours per week
Essential Functions:
Facilitate treatment and provide direct practice in the areas of group work, consumer education, treatment planning, and coordination of referrals.
Participate as a member of the multidisciplinary treatment team, performing intakes, assessments, initiating treatment interventions, and crisis intervention.
Perform clinical case management and treatment planning with consumers that experience psychiatric disorders and substance abuse disorders.
Participate in staff training and continuing education.
Take an active role in Performance Improvement projects.
Coordinate referrals and intakes from PESS, IP, PHP, State Psychiatric Hospitals, Community, and other hospital systems including but not limited to scheduling and keeping track of the status of these referrals.
Keep statistics on the outcomes of these referrals.
Facilitate referrals to other service agencies and assist clients in obtaining appropriate entitlements such as Social Security Benefits, Residential programs, and Self Help.
