The Providence Center offers a $2500 retention bonus for this position.

The Providence Center Bilingual Case Manager Highland Avenue Team is responsible for providing comprehensive case management services to clients with various needs. This includes assessing client needs, developing and implementing appropriate service plans, and coordinating support services. The bilingual role requires fluency in English and another language to provide effective communication and support to clients from diverse backgrounds.

Responsibilities

Conduct client assessments to determine service needs and identify appropriate resources.

Develop and implement individualized service plans in collaboration with clients.

Provide ongoing case management support, including crisis intervention, advocacy, and referrals to community resources.

Regularly monitor and evaluate client progress toward goals.

Maintain accurate and up-to-date client records, ensuring confidentiality and compliance with privacy regulations.

Collaborate with interdisciplinary teams to coordinate service delivery and ensure holistic care for clients.

Participate in regular team meetings, case conferences, and trainings to enhance professional knowledge and skills.

Advocate for clients' rights and access to necessary services within the community.

Support clients in navigating complex systems and overcoming barriers to achieve self-sufficiency.

Maintain positive relationships with community partners and referral sources.

Requirements

Associate's degree required; Bachelor's degree in Human Services or related fields preferred.

Spanish bilingual preferred.

Proven experience in providing case management services to diverse populations.

Strong knowledge of social services, community resources, and advocacy principles.

Excellent interpersonal and communication skills, with the ability to establish rapport and engage with clients from different cultural backgrounds.

Demonstrated ability to assess needs, develop plans, and coordinate services in a client-centered manner.

Ability to work collaboratively within a multidisciplinary team and maintain effective relationships with community partners.

Strong organizational skills, with the ability to prioritize tasks and manage multiple cases simultaneously.

Proficiency in using case management software and other relevant computer applications.
