Case Manager - Hennepin County ($1,500 sign-on bonus)
Our Case Managers enhance the quality of the case management services, maximize satisfaction and promote cost effectiveness. The successful candidate will be accountable for the full individual's care cycle; assess needs, evaluate treatment options, create care plans, coordinate care, and monitor progress to meet individual's health and human services needs.

Compensation

$45,000-52,000 (Based on experience)

$1,500 New Hire Bonus

Job Location
We are located in St. Paul, MN; however, you may be working in the community with people supported as well.

Essential Responsibilities

Develop a community support plan (goals) for each individual (typical caseload 40-45 individuals).

Assist individuals to access services, develop agreements and documentation through case notes.

Inform persons or legal guardians of service options that fit their needs.

Meet one-on-one with each individual at least twice a year to evaluate progress and adjust needs as needed.

Help identify potential providers that guide individuals towards those goals.

Evaluate and monitor services identified in the plan.

Follow case management rules (185).

Comply with Minnesota Health Care Programs regarding waiver services (e.g., CADI, BI, EW, DD), have MnChoices assessments completed and use state systems as required.

Requirements

Bachelor's Degree in Human Services, Social Work, Psychology, Sociology, or a related field.

If holding a Social Work degree, currently licensed as a Social Worker by the Minnesota Board of Social Work.

Good written and oral communication skills.

Excellent at working independently and in crisis situations.

Comfortable with high-volume paperwork.

Attention to detail and process-oriented.

Able to pass a DHS background check, valid driver's license and clean driving record.

Reliable transportation.

Work Environment
The home base will be in our office located in St. Paul, MN. Work is primarily in the community or office. Candidates will work generally Monday–Friday daytime hours, with some flexibility based on program needs. The position is full-time, non-exempt, 40 hours per week.
