The Case Manager position requires remarkable organizational and communication skills as well the ability to learn new concepts and processes quickly and manage multiple cases simultaneously.
This position is part-time, and may develop into full-time depending on employee’s performance and ability as well as organization’s needs.
Responsibilities:
• Address the financial, health and psycho-social needs of clients
• Develop care plans for clients
• Facilitate the optimal health, function and quality of life of clients
• Collaborate with the team to address all of clients' health and financial needs
• Coordinate referrals & subcontracted services
• Monitor performance of services provided by care givers and other service providers
• Provide the Conservator / Fiduciary / Trust Administrator with comprehensive updates on clients' status
Qualifications:
• Must demonstrate superb written and verbal communication skills as well as strong computer skills
• Must possess a Bachelor's Degree
• Minimum 1 year of case management experience
• Experience with Medicare, Medi-Cal & Geriatric benefits preferred
• Experience with bookkeeping or accounting is a plus
• Must pass pre-employment background check
