Job Summary: The Case Manager works in the Family Source Center (FSC) and provides case management, advocacy, support and follow-up services to customers of the FSC.
Reports To: Site Director of the FSC.
Duties and Responsibilities:
¨ Provide Case Management services to help households build a more financially secure future;
¨ Facilitates Pre-assessment and Comprehensive Assessment for all FSC customers;
¨ Works to develop an individual, holistic case plan that includes tasks, goals and a timeframe for accomplishing them;
¨ Conduct monthly, on-on-one follow-up meetings and update progress in the Service Plan;
¨ Coordinate wrap-around services, goals and action plans with the Housing Stability Advisor and Financial Coach;
¨ Conduct Multi-Benefit Screenings and assist customers in applying for public benefits;
¨ Screen customers to determine if they are eligible for referral to the City’s WorkSource or YouthSource Center for job training and placement;
¨ Screen customers to determine food security and refer for supportive service, as needed;
¨ Keeps accurate records of all client-related contacts in accordance with the FSC’s rules and regulations;
¨ Participates at case conferences, case consultations and other client-related meetings;
¨ Completes all paperwork in a timely manner;
¨ And other miscellaneous duties as assigned by Department Head.
Requirements:
Must have a BA in Psychology, Social Work/Human Services or another related field is preferred. Must support Jenesse’s mission and philosophy. Must have good communication skills. Must be able to function within a multi-cultural, multi-ethnic crisis center setting as part of a multi-disciplinary team. Must show initiative and able to work with little supervision. Must have proof of US citizenship or authorization to work, as well as reliable transportation, valid California driver’s license, and proof of state-required insurance.
Salary Range: $50,000 - $70,000 Depending upon education and experience
