Case Manager (Family Resource Center)
The City of Fremont's Human Services Department is recruiting for the Case Manager position in the Family Resource Center (FRC) Division. The FRC provides core services to a collaborative of more than 20 state, county, city of Fremont and non‑profit organizations co‑located and working together to serve individuals, families, and children in southern Alameda County. The FRC offers a friendly environment with multilingual support where residents may obtain resources, problem‑solving and benefits at one convenient location.

Job Overview
The Case Manager will perform professional and clinical social work with individuals and/or families who may have complex social and medical needs including health/mental health care, financial assistance, employment opportunities, and housing assistance. The incumbent will provide comprehensive psycho‑social assessments and develop and implement care plans. This is a 34‑hour per week position and will receive pro‑rated benefits.

Responsibilities

Manage a caseload of clients and conduct comprehensive client assessments.

Develop and implement care plans, support systems, and coordinate services for independent living.

Organize inter‑agency and family conferences and provide supportive counseling and advocacy.

Monitor and verify services, preparing written reports and case notes.

Provide phone interactions, public presentations, training programs, and publicity materials.

Assist in intern training and conduct intake assessments for other programs.

May be assigned special projects.

Qualifications

Any combination of education and experience providing the knowledge and abilities, e.g., Master’s in social work or related field with one year case work experience.

Understanding of social work principles for children, youth, families, seniors; interviewing, diagnostic assessment, counseling modalities.

Knowledge of public assistance and social service systems, health/psycho‑social needs assessment, and meaningful case management plans.

Strong communication and organization skills; ability to work with diverse backgrounds.

Valid Class C California Driver’s License.

Desirable Qualifications
Ability to speak Spanish, Chinese, or other Asian languages.
