About the Role
Join Encompass Health as a Case Manager and become a vital link between exceptional care and the transformative impact on each patient's journey. In this role you will manage resources, coordinate patient care from admission to post‑discharge, and oversee interdisciplinary plan‑of‑care decisions.
Responsibilities
Work with interdisciplinary teams, guiding treatment plans based on patient needs and preferences.
Coordinate with team to establish discharge plans and contingency plans.
Participate in planning and execution of patient discharge experiences.
Monitor patient experience regarding quality, timeliness, service appropriateness, and payor expectations.
Facilitate weekly team conferences and coordinate treatment plan modifications.
Complete case management addendums and required documentation.
Maintain knowledge of regulations, company policies, and department operations.
Review and analyze case management reports, including Key Care Indicators, and plan appropriate actions.
Understand commercial contract levels, exclusions, payer requirements, and recertification needs.
Attend ACT meetings to identify trends and collaborate on reducing ACTs.
Meet with patients/families within 24 hrs of admission and perform assessments within 48 hrs.
Educate patients/families on rehabilitation and the Case Manager role; establish communication plans.
Schedule and facilitate family conferences as needed.
Assist patients in proactive resource planning to avoid discharge delays.
Ensure compliance with orthotics and prosthetics ordering and payment regulations.
Make appropriate referrals and document post‑discharge provider/physician information.
Ensure accurate discharge and payer information in the patient record.
Participate in utilization review data collection, trend review, and resolution actions.
Participate in case management on‑call schedule as needed.
Qualifications
License or Certification:
Must be qualified to independently complete an assessment within scope of practice (e.g., RN, SW, OT, PT, ST, Rehabilitation Counseling).
Required to hold an active license if discipline licensing is required in state.
Must meet eligibility for CCM® or ACM™ certification upon entry into position or within two years.
CCM® or ACM™ certification required or to be obtained within two years of placement in Case Manager II position.
Minimum Qualifications:
For Nursing: Minimum associate degree in nursing, RN licensure with BSN preferred; diploma acceptable in states where diploma is minimum requirement.
For other licensed or certified health care professionals: Minimum bachelor’s degree; graduate degree preferred.
2 years of rehabilitation experience preferred.
