General Description
The Case Manager is responsible for resource assessment to identify client needs and establish connections/rapport within the community to provide myriad resources and connection to services. Case managers will be responsible for service planning and service plan implementation & coordination, as well as monitoring and follow-up on these services. Case manager is expected to be active in aftercare planning, crisis intervention and third-party requests. Case manager is expected to perform intake and assessment as needed.
