Home Based Case Managers are responsible for the ongoing monitoring and coordination of services. Those services include the personal care, health care, and other supportive services, provided to clients in their own homes. That will enable clients to continue living at home, and to maintain as much independence as possible. Case Managers must contact the client via the phone the first two months of the quarter and visit the participant the last month of the quarter. Ability to work independently with minimum supervision. Must be self-motivated and have strong organizational skills.
Minimum Requirements: Individuals with a Bachelor’s or Master’s degree in a health or human services field from an accredited college or university, have at least two (2) years of assessment and care planning experience with clients.Or be a Registered Nurses currently licensed by the state of South Carolina or by a state that participates in the Nursing Compact. Or be a Certified Care Managers with two (2) years of assessment and care planning experience with clients.
