Role Description
This is a full-time, on-site role for a Case Manager at The Advocaci Group, PLLC, located in Dallas, TX. The Case Manager will be responsible for managing and coordinating all aspects of case management, including conducting client assessments, creating case plans, monitoring client progress, and providing support and resources to clients. The role also involves liaising with healthcare providers, legal professionals, and other relevant organizations. Additionally, the Case Manager will be responsible for maintaining accurate records and ensuring compliance with all relevant policies and regulations.
