Case Manager – Part Time

Location: Hummelstown, PA
Schedule: Per Diem, weekdays required, some weekends.
Compensation: $40.00 per hour

Responsibilities
Maintains tools to enhance operations such as discharge calendar and team conference schedules.
Manages daily workload, additional assignments, routine clerical duties (photocopy, filing, faxing).
Greets patients, families, vendors, provides basic hospital information.
Communicates with hospital personnel, prepares information for team conferences, daily huddles, family training sessions, care partner meetings.
Obtains authorization of services for precertification/recertification as delegate; communicates insurance and benefit issues.
Ensures adequate supplies and equipment; maintains and organizes department resources.
Schedules follow‑up appointments, coordinates transportation, performs follow‑up calls.
Faxes necessary information for discharge services to sub‑acute facilities, home health, outpatient, follow‑up physicians, DME.
Utilizes appropriate computer systems for billing, case management documentation, census, reports.
Coordinates with other departments to ensure positive fiscal management outcomes for patients.
Minimum Qualifications
High School diploma or equivalent required.
Proficiency in office skills, typing and computer skills, use of office equipment required.
Minimum of one year of administrative/secretarial experience in a medical setting required.
Preferred Qualifications
Previous experience within a case management department and/or rehabilitation setting preferred.
Associate’s Degree preferred.
Post‑offer employment testing (POET) will be completed before first day of work.
